Computer Studies Division SY 2008-2009 Enrollment Guidelines for Evaluator/Coordinator
1.) Check if student is REGULAR or IRREGULAR. a.) If student is regular, proceed to # 2 b.) If student is irregular, identify the deficiencies of the student using his/her folder and write them on the evaluation form, and refer to Coordinator.
2.) Check the previous status using the semester evaluation list. a.) If student is not on probation: i.) If WPA is >= 80 and MSCA >=82 and with no grades 75 - OK to enroll ii.) If WPA is < 80 or MSCA < 82 and with no grades below 75 - 1st Probation iii.) If failed 1 MAJOR SUBJECT - 1st Probation iv.) If failed 2 MINOR SUBJECTS - 1st Probation v.) If failed 2 MAJOR SUBJECTS - Advised to Shift
b.) If student is on 1st probation: i.) If WPA is >= 80 and MSCA >= 82 and with no grades below 75 - lifted 1st probation, OK to enroll ii.) If WPA is < 80 or MSCA < 82 and with no grades below 75 - 2nd Probation iii.) If failed 1 MAJOR SUBJECT - 2nd Probation iv.) If failed 2 MINOR SUBJECTS - 2nd Probation v.) If failed 2 MAJOR SUBJECTS - Advised to Shift
c.) If student is on 2nd probation: i.) If WPA is >= 80 and MSCA >= 82 and with no grades below 75 - lifted 2nd probation, OK to enroll ii.) If WPA is < 80 or MSCA < 82 and with no grades below 75 - refer to Coordinator iii.) If failed 1 MAJOR SUBJECT - Advised to Shift iv.) If failed 2 MINOR SUBJECTS - Advised to Shift v.) If failed 2 MAJOR SUBJECTS - Advised to Shift
d.) If student is on last and final probation or special probation: i.) If WPA is >= 80 and MSCA >= 82 and with no grades below 75 - lifted probation, OK to enroll ii.) If WPA is < 80 or MSCA < 82 and with no grades below 75 - Advised to Shift iii.) If failed 1 MAJOR SUBJECT - Advised to Shift iv.) If failed 2 MINOR SUBJECTS - Advised to Shift v.) If failed 2 MAJOR SUBJECTS - Advised to Shift
3.) Indicate the NEW STATUS of the student on the evaluation form.
4.) Write the deficiencies of the student on the evaluation form if applicable.
